Our Services

Aging Life Care is a holistic, client-centered approach to assisting older adults or others in active planning for future life care needs. Guidance is also provided to individuals facing ongoing healthcare challenges to ensure appropriate steps are taken to establish dependable quality care and other appropriate support services; thus reducing worry, stress and time off work for family caregivers through:

 

  • Assessment/Identification of life care challenges

  • Recommendation of appropriate services

  • Planning and problem-solving

  • Monitoring selected service providers

  • Education and advocacy

  • Family caregiver guidance

  • Long-distance support

 

Our staff is focused on assisting clients in attaining their maximum functional potential. The individual's independence is encouraged, while safety and security concerns are also addressed. We have extensive knowledge about the costs, quality, and availability of resources in the Tampa Bay area and can assist in a variety of areas:

 

  • Residential - Optimizing independence and support services within a client's home setting, evaluating a client's level of care and providing appropriate residential setting recommendations, and/or coordination of transitional care assistance upon discharge from the hospital to a rehabilitation setting.

 

  • Safety/Security/Social Support - Assessment of client support systems in place through family, home care or other agencies; evaluation of home safety and need for adaptations within the home environment to maximize independence and aging in place; recommendation of technologies to enhance security or safety; observing changes and potential risks of explotation or abuse. â€‹

 

  • Home Care Services - Determining types of services needed by the client and assisting in engaging and monitoring these services, if needed.

 

  • Medical Management - Attending doctors appointments, facilitating communication between doctor, client and family.

 

  • Financial - Assessment of financial resources available through long term care and other health insurance plans, or government entitlement programs to maximize benefits and financial stability of our clients. We also assist with bill paying, coordination of insurance benefits, consulting with the client's accountant, and other financial advisors.

  • Legal - Consultation and referral to elder law attorneys for Advance Directive planning: Trusts, Wills, Living Wills, Durable Powers of Attorney, Health Care Surrogate or Pre-Need Guardian designation.

  • Long-Distance Care - Coordinating the care of a loved one for families that live at a distance; including crisis management 24/7.
